This lecture presents a diagnostic-first approach to managing intrinsically discoloured anterior teeth. Through four clinical cases, Dr Adnill George Kock demonstrates how to differentiate between necrotic pulp, calcific metamorphosis, and internalised discolouration, and how each diagnosis drives a different treatment strategy. Techniques covered include the inside-out bleaching method using low-dose carbamide peroxide, intracoronal bleaching, CEJ-guided barrier seal placement, and prosthetic masking with opaque composites when tooth structure is too compromised for conservative management.

Aims & Objectives:

1) Differentiate between intrinsic discolouration, internalised discolouration, and calcific metamorphosis to guide treatment planning.
2) Understand the inside-out bleaching technique and the rationale for low-dose carbamide peroxide over high-concentration hydrogen peroxide.
3) Apply CEJ-guided coronal barrier seal placement to prevent external cervical resorption during intracoronal bleaching.
4) Identify when minimally invasive bleaching is appropriate versus when prosthetic masking is the better clinical option.
