Class I restorations are among the most frequently performed procedures in dentistry, yet they are often underestimated in terms of their biomechanical and adhesive complexity. In this lecture, Dr Abdullah Bazrgan presents a biomimetic approach to the diagnosis, preparation, and restoration of occlusal cavities, grounded in an in-depth understanding of natural tooth structure and stress distribution. Drawing on the teachings of his mentor Dr David Alleman and contemporary adhesive science, he explains how preservation of critical tooth structure, control of C-factor, and optimisation of the hybrid layer directly influence long-term success. Through clinical cases and evidence-based concepts, this lecture reframes the “simple” Class I restoration as a procedure that demands precision, strategy, and respect for the natural tooth structure.

Aims & Objectives:

1) Understand how enamel, dentin, and the dentino-enamel junction contribute to stress distribution and fracture resistance.
2) Recognise the biomechanical consequences of occlusal cavity preparation and identify critical tooth structures that must be preserved.
3) Apply biomimetic strategies to improve bond strength and reduce polymerisation stress in high C-factor Class I restorations, using concepts such as Decoupling With Time.
4) Integrate evidence-based protocols for caries removal, adhesive selection, and composite layering to enhance restoration longevity.
