The quality of our endodontic treatment depends heavily on the environment we create before it begins. Endodontically treated teeth are often extracted because of coronal leakage, recurrent caries or fracture, yet restorability is still commonly judged after the endodontic treatment is complete. Restoratively driven endodontics reverses that, dismantling and rebuilding the tooth first so that the canals are cleaned, shaped and obturated in a sealed, isolatable, four-walled environment. The pre-endodontic build-up is as much a diagnostic procedure as a restorative one, because only once the old restoration and the caries are removed can we judge how much sound dentin remains and whether the tooth is restorable. Dr Adnill George Kock shares the literature and the reasoning behind this approach, with cases ranging from severely broken down molars to crowned teeth, retreatments and paediatric patients.
