Selective caries removal has become the preferred approach for vital permanent teeth with deep caries, but Dr Michaela Jaresova believes it is often interpreted too broadly, with carious dentin left where the evidence does not support it. She takes a balanced, evidence-based look at the technique, setting out clearly where it is appropriate and where it is not. Her approach centres on the Peripheral Seal Zone: a clean periphery of sound enamel and dentin to which we can predictably bond, together with a caries removal endpoint that avoids exposing the pulp. Through her own clinical cases, she shows how to make these decisions confidently in daily practice. She also explains why a symptom-free tooth is not proof of a healthy pulp, and how she discusses this honestly with her patients.
