Dr Pardis Doosti

Fibre Placement Strategies in Adhesive Dentistry.

presented by:
Dr Pardis Doosti

Dr Pardis Doosti presents a structured guide to fibre selection and placement in adhesive restorations. Starting with the biomechanical principles that underpin fibre use, she explains how natural tooth structures like the dentinoenamel complex inform our restorative approach. Dr Doosti walks through each intracoronal fibre indication: stress-reduced direct composites, biobase construction, and post-core systems for endodontically treated teeth. For each scenario, she demonstrates specific placement designs, including wallpaper, C-shape, axial wall reinforcement, pulpal floor protection, and crack bridging with continuous fibres. She also compares the mechanical properties and handling of glass fibres and polyethylene fibres, offering practical guidance on which to choose based on the clinical situation. Dr Doosti also addresses when fibres are not indicated, helping clinicians avoid unnecessary reinforcement in cases where sound tooth structure provides adequate support.

Aims & Objectives:

1) Understand the biomechanical rationale for fibre reinforcement, including how fibres mimic dentin and dentinoenamel complex functions in adhesive restorations.
2) Learn specific fibre placement designs for intracoronal applications, including wallpaper, C-shape, axial wall, pulpal floor, and crack bridging configurations.
3) Compare the properties and clinical applications of glass fibres, polyethylene fibres, and short fibre-reinforced composites to select the appropriate material for each scenario.
4) Identify when fibre reinforcement is and is not indicated, based on residual tooth structure, cavity depth, and restorative approach.
